Ambulance
If medically necessary; the insured is required to pay a user's fee of $45.00 for land or air ambulance (including the cost of connecting land ambulances). The user's fee does not apply to an insured person who is transferred from one hospital to another, or who is receiving home care etc.
The use of air ambulance requires prior approval from Ontario Emergency Health Services.
If not medically necessary or the patient does not have a valid Ontario Health Card or Health 65 Card a $240 charge is the average cost of a land ambulance trip in Ontario. The patient is charged the full cost of any air ambulance flight that is not medically necessary or not covered and the patient must also pay the $240 for any land ambulance used.
Laboratory - other than in Hospital
Services specifically and expressly authorized by a physician who has clinically assessed the patient and are performed in a laboratory licensed under The Public Health Act or a public health laboratory approved as a related health facility.
X-rays - other than in Hospital
X-rays for diagnostic and treatment purposes requested by a
physician are covered.
